Jacobson Innovation Award (Established 1994)

The prestigious Jacobson Innovation Award honors living surgeons who have been innovators of a new development or technique in any field of surgery. The award is made possible through a gift from Julius H. Jacobson II, MD, FACS, and his wife, Joan. Dr. Jacobson is a general vascular surgeon known for his pioneering work in microsurgery. Nominees considered for the Award should represent surgeons who have developed the most impactful and highly influential surgical innovations for the various surgical specialties over the past ten years. Also, the nominees considered must be the original and true innovator of a pioneering technique or procedure. The Award is presented during a dinner held in the recipient’s honor in June in Chicago, IL.

Submit a Jacobson Innovation Award Nomination Form and provide the following information:

  • Contact information for the nominator and nominee.
  • Upload a minimum of two letters of support which list accomplishments of the nominee and how they meet the criteria for the award or honor.
  • Upload a current curriculum vitae of the nominee.
  • Upload a short biography of the nominee.
  • Please keep the nomination confidential from the proposed nominee.
