Esophagectomy in cT4 esophageal adenocarcinoma poses a technical challenge and its benefit has not been well established. This propensity-score matched NCDB analysis of patients with cT4 nonmetastatic esophageal adenocarcinoma shows a survival benefit to esophagectomy in cT4a and cT4b esophageal adenocarcinoma compared with nonoperative management.

PV thrombosis, resulting in cavernous transformation and peripancreatic varices, is a rare but possible complication of pancreatic adenocarcinoma in the setting of ERCP-induced pancreatitis.
