Submitting Documents Post-Site Visit

For cancer programs anticipating a CoC accreditation site visit, the Pre-Review Questionnaire (PRQ) will close 60 days before the scheduled site visit date. This allows the site reviewer time to review the completed PRQ and supporting documents.

On occasions where the site reviewer requests programs to upload missing or additional documents either on the day of the site visit (when the PRQ re-opens to programs) or during the 3 days post-site visit, programs should follow these instructions:

  • Upload the requested documents directly into the PRQ under the appropriate standard or under the “Other” section.
  • Once the documents have been uploaded, move to the last page of the PRQ and click on “Submit” again. If “Submit” is not clicked, the newly uploaded documents are not accessible to the site reviewer or ACS Cancer Programs staff.

Documents that support compliance to standards that are uploaded post-site visit to the “Site Visit Documents” section of the PRQ page in QPort will not be reviewed as part of the site visit.

  • Reminder: This field on the PRQ page is intended only for communication between site reviewers and programs before the site visit takes place. This includes communication on travel or contact information, agenda, patient lists (with no PHI), and so on.
  • If documents were uploaded in this field before the site visit at the request of your site reviewer, the documents must be re-uploaded into the appropriate spot in the PRQ when the PRQ re-opens on the day of the site visit.

 

These steps should also be followed when submitting documents in the Corrective Action PRQ.

  • The Corrective Action documents are to be uploaded directly into the PRQ under the appropriate standard.
  • Once the documents have been uploaded, move to the last page of the PRQ and click on “Submit” again. If “Submit” is not clicked, the newly uploaded documents do not appear for site reviewers and ACS Cancer Programs Staff to access.
