Using the PRQ Site Visit Documents Link in QPort

As part of the CoC pre-review process, additional documentation is occasionally requested by the site reviewer after the Pre-Review Questionnaire (PRQ) has closed 60 days before the scheduled site visit. The PRQ will re-open on the day of the site visit and remain open for 3 business days following the visit.

  • Once the PRQ re-opens on the day of the site visit, the requested documents should be uploaded directly into the PRQ under the appropriate standard or under the “Other” section. Requested documents related to standards are not to be uploaded to the “Site Visit Documents” link in QPort. This section is not considered part of the site review and will not be reviewed as part of the review (this also applies to corrective action uploads).
    • The Site Visit Documents link on the PRQ page is intended only for communication between site reviewers and programs before the site visit takes place. This includes communication on travel, contact information, agenda, patient lists (with no Protected Health Information), etc.
    • If documents were uploaded in this field before the site visit at the request of your site reviewer (for communication purposes) and are supporting compliance to a standard, the documents must be uploaded again under the appropriate standard in the PRQ when it re-opens on the day of the site visit.
  • Once the documents have been uploaded in the PRQ, move to the last page and click on “Submit” again. If “Submit” is not clicked, the newly uploaded documents will not appear for site reviewers and ACS Cancer Programs Staff to assess.

Submitting Corrective Action Documents

  • The Corrective Action documents are to be uploaded directly into the Corrective Action PRQ under the appropriate standard. If the standard in question does not have an upload section on its page, please contact CoC@facs.org for further assistance.
  • Once the documents have been uploaded, move to the last page of the Corrective Action PRQ and click on “Submit” again. If “Submit” is not clicked, the corrective action documents do not appear for site reviewers and ACS Cancer Programs Staff to assess.
