How do I apply?

First, check that your center complies with all MBSAQIP standards (including the volume standard) before you move forward with your application. When you are ready, fill out the initial application.

What is the initial accreditation process?

Once a center’s application is reviewed, the contract has been finalized, and program fee had been processed, the site’s Prereview Questionnaire (PRQ) is released. The center must submit ALL their materials within 6 months. If materials are not submitted within this timeframe, MBSAQIP reserves the right to withdraw your center from the accreditation process.

When will I receive my Pre-Review Questionnaire (PRQ) if my center is applying for accreditation?

Once a center’s application is reviewed, the contract has been finalized, and program fee has been processed, the site’s Prereview Questionnaire (PRQ) is released. In most cases, you can expect to receive your PRQ 1-2 months after initial application.

After application submission, you will be contacted by ACS staff to complete your contracts and pay the programmatic fee. Next, site contacts and designation level selection will be requested from an ACS program coordinator. Once these items are complete, you will begin working on your PRQ.

When do site visits occur?

Site visits for initial centers are generally scheduled for about 5-7 months after the PRQ is issued but can be dependent on other factors.

Are site visits in-person or virtual?

At present, initial sites undergo in-person site visits. Renewal center site visits are completed via a virtual format.

How do I know when to renew my accreditation?

Six months before the end of your site’s accreditation, a Renewal Notice will be sent from MBSAQIP to the primary contact on file. Please be sure this contact information is current.

Who is the primary contact at my site?

If you are unsure of your primary contact, please reach out to mbsaqipaccess@facs.org and we will assist you.

What is a Metabolic and Bariatric Surgery Clinical Reviewer (MBSCR)?

An MBSCR fulfills case abstraction duties, enters long-term follow-up data on patients, and fulfills requests for outcomes data and reports.

Can a center have more than one MBSCR?

Yes. MBSAQIP recommends a backup in place in the event of leave, vacation, etc.

Who can access the Semi-Annual Report (SAR)?

Anyone with a data registry role (MBSCR, MBS QI User) can access the SAR.
